WRAPAROUND. PROCESS OF CHANGE.
WRAPAROUND is a process of change that partners families, youth and professionals in a collaborative effort to develop a customized care management plan for youth with serious emotional or behavioral health issues.

COLLABORATIVE EFFORT. FAMILY FOCUSED.
The Care Coordinator and Family Support Provider work closely with referred families to identify a Family Team consisting of family members and other support persons, such as neighbors, friends or counselors who care about and know the family best, and other service providers such as job services, health services, school services and more.

CUSTOM APPROACH. YOUTH CENTERED.
The Wraparound staff and the Family Team, then develop a customized plan focusing on (1) meeting the youth’s physical, emotional, intellectual, educational, social, cultural and family needs, (2) mobilizing available resources (3) identifying goals.

EMPOWERMENT. RESULTS ORIENTED.
Wraparound empowers the youth and their families that results in:

decreased out of home placements

improved school performance

promoting positive emotional and behavioral functioning

decreasing interactions with the legal system

enhancing the overall quality of life for the participants
